Do they just fall out sometimes if you're not paying attention, or is it some kind of astral projection gone wrong? Or was this one sold for some sort of financial gain and then simply misplaced by the new owner? For that matter, maybe this is a new soul that was on its way to its first body, and it missed because of a bad sense of direction?

Your body twitches for a moment as it makes space for another bit of spiritual essence and a strange voice enters your head with a friendly "Hi there!" You're sure that given enough time you'll be able to convince this soul to leave, but in the meantime you're going to have to learn to cooperate. You get an effect, Tangled Up in Two.
