Before investigating the Sanatorium:

You return to the creepy old man and his creepy old house, but no matter how hard you pound on the door, he refuses to answer. I guess he's done with you until you build whatever it was he wanted from the Sanatarium.

Before assembling and equipping the dreamer's clock:

You return to the creepy old man and his creepy old house, but no matter how hard you pound on the door, he refuses to answer. I guess he's done with you until you build whatever it was he wanted from the Sanatarium.

After assembling the dreamer's clock and choosing it as your means of transportation:

You return to the creepy old man and his creepy old house, knocking on the door. He answers almost immediately, coughing and wheezing, "Ah ... finally ... come in ... come in ..."

You follow him back inside his decrepit house, going back to that sitting room. It looks much the same as before, except that it might have a few more bottles than last time. He motions for you to sit while he all but collapses in his creaking rocking chair.

Silently, almost ritualistically, he pulls a pipe from his clothes, patting it out, tamping in fresh tobacco, and then lighting it. He takes several puffs before leaning forward and fixing you with his unnerving gaze, "Now that ye're ready, I'll tell ye that there's more than jus' the crimes ye've been fighting... out in the seas... deep in space... up in the mountains... and in the very crux of madness... there are things beyond... things of unspeakable evil..." He coughs and hacks a little and continues, "As ye fight... ye may come across certain... artifacts... bring them t' me... in exchange, I'll give ye' certain... trinkets... that ye may find useful..." As before, he turns his attention back to the fire, seemingly done talking to you.

After finishing the quest, but before finding notable artifacts from the now accessible areas:

You return to the creepy old man and his creepy old house, knocking on the door. He answers almost immediately, coughing and wheezing, "Ah... finally... come in... come in..."

You follow him back inside his decrepit house, going back to that sitting room. It looks much the same as before, except that it might have a few more bottles than last time. He motions for you to sit while he all but collapses in his creaking rocking chair.

He looks you over again, but shakes his head. "I cannot do more for ye until ye bring me ... artifacts ... from beyond."
