Ultra-rares are exceedingly rare items that have an exceptionally low chance of dropping. As far as anyone currently knows, there are no "tricks" or bizarre requirements to getting an ultra-rare, they just have a very, very low chance of happening. There are currently 7 known ultra-rare items in the game:
Abaddon charm from the Fiendish Pit
Boomstick from the Guild for Imaginative Metachronism
Ebon Eye of the Deep from the Porcelain Bay
Filthy sock puppet from Investigating the trash bin out back in Somerset
SQUID disc: The Best Memory from the Cube Theater
Ultimate Aviator Goggles from Flying above Twilight
Shimmering chaos from Azathoth in The Farthest Reaches of Insanity
